A newborn baby who died at the Regina General Hospital was accidentally smothered by her mother, according to a coroner's report.
Cambria Tammie-Leigh Shuba died March 20 while she was in bed with her mother.
About five hours after Shuba was born, a nurse discovered the baby's mother had rolled onto her and that the newborn wasn't breathing.
Last November, the Regina General Hospital opened its new mother-baby unit with 36 private rooms. A general nursery where all babies used to be kept is gone.
The hospital's procedure was to keep babies in their mothers' rooms to encourage bonding and follows guidelines introduced by Health Canada in 2000.
However, the tragic death prompted the Regina Qu'Appelle Health Region to develop a new policy to prevent mothers and babies from sleeping together.
In her report, the coroner noted that the unit did not have proper nurse call and code button systems. The health region has said it will make changes to those systems.
An external review of the hospital's procedures is currently underway.
